All residents of Illinois 5 years old and over are eligible for the Pfizer vaccine, and 18 years and older for the Moderna and Johnson & Johnson vaccines. APPOINTMENTS FOR IN-HOME VACCINATIONS Call the Village of Arlington Heights at 847-368-5760 and ask for Nursing Services to schedule an in-home vaccine. If you are not a resident of Arlington Heights, but live in Cook County, fill out this form or call 1-833-308-1988, 7 a.m. to 7 p.m. Monday to Friday to request an in-home vaccine.

Northwest Community Healthcare: Appointments are available by phone at 847-618-0242 or online at vaccineschedule.nch.org/mychart/covid19# . Also, NCH has opened a walk-in COVID-19 first dose vaccination clinic for people 12 years old+ on Mondays and Wednesdays, 9:00 a.m. - 7:00 p.m. at 3060 Salt Creek Road, Arlington Heights. If you are over 18 years old, you can receive a vaccine at one of the Immediate Care Centers by appointment only. If you are homebound, call to schedule an in-home vaccine.
Osco: Register online or walk-in.
Walgreens: You will need to log in to your Walgreens account to access the calendar or call 1-800-925-4733 to make an appointment. Walgreens offers same-day appointments and walk-in vaccinations.
Walmart: Must create an online account to make an appointment. Walmart and Sam's Club also offer walk-in vaccinations.
